Targeting skeletal and cardiac muscle is a cornerstone of gene therapy for muscular dystrophies (like DMD) and metabolic disorders. Because muscle is the largest organ system by mass, the primary challenge is achieving systemic distribution while minimizing the “liver sink” effect.
| Serotype | Primary Target Cells | Preferred Route | Key Characteristics & Notes |
| AAV1 | Skeletal Muscle Fibers | IM | The Local Champion. • Historically the first choice for direct muscle injection. • Excellent for local secretion of proteins into the blood (e.g., Alpha-1 antitrypsin). |
| AAV6 | Skeletal Muscle, Cardiomyocytes | IV / IM | High Potency. • Closely related to AAV1 but significantly better at crossing the vascular barrier to reach muscles from the bloodstream. |
| AAV8 | Skeletal & Cardiac Muscle | IV | Global Distribution. • Very efficient at systemic muscle transduction, but its extreme affinity for the liver often limits the “net” dose that reaches the muscle. |
| AAV9 | Skeletal Muscle, Heart, CNS | IV | The Systemic Standard. • Widely used for systemic muscle diseases. It has a high capacity to escape the vasculature and penetrate muscle tissue. |
| AAV7 | Skeletal Muscle | IV / IM | Similar performance to AAV1/AAV6 in muscle; often used in preclinical NHP (Non-Human Primate) studies but less common in human trials. |
These are designed to increase “Musculotropism” (muscle-seeking) while “Detargeting” (avoiding) the liver.
| Variant | Primary Target Cells | Preferred Route | Key Characteristics & Notes |
| MyoAAV | Skeletal Muscle (All Types) | IV | The New Benchmark. • Engineered with a unique peptide (RGD-motif) that binds to integrins on muscle cells. • 10–50x more potent than AAV9 in muscles while significantly avoiding the liver. |
| AAV-B1 | Skeletal Muscle, Heart, Brain | IV | A bioengineered variant that shows broad muscle transduction and reduced liver sequestration compared to wild-type AAV9. |
| AAV-SLB | Skeletal Muscle | IV | “Skeletal-Muscle-Bypass.” • Specifically engineered to reduce liver uptake. It shows high specificity for skeletal muscle over other organs. |
| AAV-myo | Muscle Fibers | IV | A variant developed via directed evolution to specifically improve transduction of dystrophic muscle tissue (which is harder to infect than healthy muscle). |

In diseases like DMD, muscle tissue is not “normal”—it is characterized by inflammation, fibrosis (scarring), and fatty replacement.
