Targeting the kidney with AAV is notoriously difficult compared to the liver or eye. The kidney’s complex filtration system and the presence of the glomerular filtration barrier (GFB) make it hard for vectors to reach specific segments like the podocytes or the tubular epithelium.
| Serotype | Primary Target Cells | Preferred Route | Key Characteristics & Notes |
| AAV9 | Podocytes, Glomerular Cells | IV / Renal Artery | The Best Natural Performer. • One of the few natural serotypes capable of transducing the kidney at a measurable level systemically. • Primarily targets the glomerulus (podocytes) but efficiency is still relatively low compared to other organs. |
| AAV2 | Tubular Epithelium | Retrograde | Localized Efficiency. • Very poor via IV due to liver sequestration and poor penetration. • If delivered via the ureter (retrograde), it can effectively reach the renal tubular cells. |
| AAV1 / AAV6 | Vascular Endothelium | IV | Mostly restricted to the renal vasculature; they rarely penetrate into the deeper functional units (nephrons) from the bloodstream. |
| AAV8 | Inefficient in Kidney | N/A | While excellent for the liver, AAV8 shows almost negligible transduction in the kidney compared to AAV9. |
Because natural AAVs are poor kidney vectors, engineering is the primary focus for renal gene therapy.
| Variant | Primary Target Cells | Preferred Route | Key Characteristics & Notes |
| AAV-Anc80 | Tubular Cells, Podocytes | IV / Renal Artery | Ancestral Potency. • A synthetic ancestral capsid that significantly outperforms AAV9 and AAV8 in reaching the renal cortex and tubules. • High stability and lower pre-existing immunity in humans. |
| AAV-PHP.B | Glomerular Endothelium | IV | Originally designed for the Blood-Brain Barrier (BBB), this capsid also shows enhanced ability to transduce renal vascular and glomerular cells compared to wild-type AAV9. |
| AAV-DJ | Tubular Cells | Retrograde | Highly efficient in vitro; in vivo it performs well if delivered directly to the renal pelvis, but like others, it fails to hit the kidney effectively via IV. |
| AAV-B1 | Multiple Nephron Segments | IV | A bioengineered variant that shows broad transduction across the kidney, including the tubules, which are usually shielded from systemic vectors. |

The kidney is a biological sieve. To get a gene into a tubular cell from the blood, the AAV must pass through:
The “Liver Problem” in Kidney Research:
Because the kidney is so difficult to target, researchers often use Kidney-Specific Promoters (like Nphs1 for podocytes or Ksp-cadherin for tubules). Even if the AAV ends up in the liver (which it will), the therapeutic protein is only produced in the kidney cells.
