Targeting the gastrointestinal (GI) tract with AAV is a significant challenge due to the rapid turnover of the intestinal epithelium (cells are replaced every 3–5 days), the thick protective mucus layer, and the presence of digestive enzymes and harsh pH levels.
| Serotype | Primary Target Cells | Preferred Route | Key Characteristics & Notes |
| AAV9 | Enteric Neurons, Smooth Muscle | IV | The Neuro-Muscular Specialist. • The most effective natural serotype for reaching the Enteric Nervous System (ENS) via systemic injection. • Transduces the myenteric plexus but poorly targets the epithelium from the blood side. |
| AAV6 | Intestinal Epithelium | Intraluminal | The Epithelial Performer. • Shown to have a higher affinity for the apical (luminal) surface of intestinal cells compared to AAV2 or AAV8. • Still requires “muco-active” agents to bypass the mucus layer. |
| AAV8 | Lamina Propria, Myofibroblasts | Systemic | Generally poor at reaching the epithelium, but can transduce deeper mesenchymal cells in the intestinal wall if high titers are used. |
| AAV2 | Epithelium (Limited) | Intraluminal | Historically used, but significantly hindered by heparin-binding, which causes it to get stuck in the mucus and glycocalyx. |
Engineered capsids for the gut focus on resisting degradation and improving penetration through the mucus.
| Variant | Primary Target Cells | Preferred Route | Key Characteristics & Notes |
| AAV-PHP.S | Enteric Neurons (ENS) | IV | Superior ENS Targeting. • A variant of AAV9 engineered for the peripheral nervous system. • Shows significantly higher efficiency in transducing neurons across the entire length of the GI tract compared to wild-type AAV9. |
| AAV-DJ | Intestinal Epithelium | Intraluminal | High Infectivity. • A synthetic hybrid that exhibits very high transduction rates in intestinal organoids and in vivo when delivered locally, though susceptible to the “washout” effect. |
| AAV-iHLS1 | Intestinal Epithelium | Oral / IV | Intestine-Homing. • A newer engineered capsid designed to bypass the liver and home specifically to the small intestine and colon. |

The biggest obstacle in intestinal gene therapy is not just the capsid, but the clock:
