| Promoter | Target Tissue | Size (bp) | Origin | Key Characteristics |
| MCK | Skeletal & Cardiac | ~1.3 kb | Mouse/Human | Muscle Creatine Kinase. High specificity but relatively large for AAV packaging. |
| tMCK | Skeletal & Cardiac | ~720 bp | Synthetic | Truncated MCK. Optimized for AAV; removes non-essential regions to save space while maintaining high potency. |
| CK8 / CK7 | Skeletal & Cardiac | ~500–800 bp | Human | Versions of the Creatine Kinase promoter often used in tandem with enhancers (like the MHCK7 chimeric version). |
| Desmin | All Muscle | ~1.0 kb | Human/Mouse | Active in skeletal, cardiac, and smooth muscle. Useful for systemic muscle targeting. |
| α-Actin (ACTA1) | Skeletal Muscle | ~2.0 kb | Human | Highly specific to skeletal fibers; very strong but takes up significant “real estate” in the vector. |
| MHC (α-MHC) | Cardiac Muscle | ~1.0–5.5 kb | Mouse/Rat | Myosin Heavy Chain. The $\alpha$-isoform is specific to the heart; the $\beta$-isoform is skeletal. |
1. The tMCK and MHCK7 Advantage
In modern AAV design, the full-length MCK promoter is rarely used because it is bulky. Instead, researchers use tMCK (truncated) or MHCK7. MHCK7 is a chimeric promoter (~770 bp) that combines the MCK enhancer with the $\alpha$-MHC promoter, offering superior strength in both the heart and skeletal muscle compared to either native promoter alone.
2. Liver Sequestration
A major challenge with “muscle-specific” promoters is that AAV often naturally traffics to the liver. While promoters like Desmin or MCK are silent in the liver, the sheer volume of viral particles that end up there can cause “leaky” expression. To combat this, researchers often add miRNA binding sites (e.g., miR-122) to the 3′ UTR of the vector to actively silence any transgene mRNA that ends up in liver cells.
3. Smooth Muscle vs. Skeletal
If your target is the vasculature or the bladder, Desmin is a better choice than ACTA1 or MCK, as Desmin is one of the few promoters that remains highly active in smooth muscle cells.
