1. Deep Lung Targeting with SP-C Alveolar Type II cells are responsible for producing lung surfactant and acting as progenitor cells for the alveolar epithelium. The SP-C promoter is the most common tool for targeting these cells. While the full promoter can be large, truncated versions (approx. 1.5 kb) are frequently used in AAV research to deliver genes that correct surfactant metabolism disorders.

2. Airway Clearance and FOXJ1 For diseases like Primary Ciliary Dyskinesia (PCD), the FOXJ1 promoter is essential. By driving expression in ciliated epithelial cells, researchers can attempt to restore ciliary beat frequency. Because FOXJ1 is a transcription factor that regulates the “ciliogenesis” program, its promoter is highly specific to cells that possess motile cilia.

3. CC10 and the “Clara Cell” Factory The CC10 (or Club Cell Secretary Protein) promoter is highly active in the bronchiolar epithelium. It is often used in studies involving airway inflammation and asthma. Because Club cells are highly secretory, they can be used to produce and secrete therapeutic proteins directly into the airway surface liquid.

4. Viral Tropism Coordination Lung targeting is often limited by the physical barrier of mucus. Researchers frequently pair these promoters with specialized AAV capsids like AAV6, AAV-PHP.B, or synthetic variants like AAV2-retro (if administered via the trachea) to bypass the mucosal layer and reach the underlying epithelial cells.
