| Promoter | Target Cell Type | Size (bp) | Origin | Key Characteristics |
| cTnT | Cardiomyocytes | ~400–500 bp | Human / Rat | Cardiac Troponin T. Very popular for AAV due to its small size and high specificity for cardiac muscle. |
| cTnI | Cardiomyocytes | ~2.1 kb | Human | Cardiac Troponin I. Highly specific but much larger than cTnT; used when robust, long-term expression is needed. |
| α-MHC | Cardiomyocytes | ~1.0–5.5 kb | Mouse | Alpha-Myosin Heavy Chain. Drives high expression levels. Truncated versions (~1kb) are common in AAV. |
| MLC2v | Ventricular cells | ~250–500 bp | Human / Rat | Myosin Light Chain 2v. Restricts expression specifically to the ventricles, sparing the atria. |
| NPPA (ANP) | Atrial cells | ~500 bp – 3 kb | Human / Mouse | Natriuretic Peptide A. Primarily active in the atria; can be upregulated in ventricles during heart failure. |
1. Chamber Specificity (Atria vs. Ventricles) Standard muscle promoters like Desmin or MCK will target the entire heart. However, if your therapy is for atrial fibrillation, you might use NPPA. Conversely, for ventricular-specific conditions like certain hypertrophic cardiomyopathies, MLC2v is the preferred choice to avoid affecting atrial function.
2. The “cTnT” Efficiency The cTnT promoter is often the “hSyn equivalent” for the heart. At roughly 480 bp, it provides excellent cardiac restriction with a high “transgene-to-promoter” ratio, making it ideal for large cardiac genes like MYBPC3.
3. Liver De-targeting Even with a cardiac-specific promoter, AAV9 or AAV-Rh10 (common cardiac serotypes) will heavily infect the liver. To ensure absolute safety, researchers frequently combine these cardiac promoters with miR-122 binding sites in the 3′ UTR to suppress any “leaky” liver expression.
