AAV Tropism for Specialized Tissues

Beyond the primary organs already discussed, AAV vectors are being engineered to target more specialized tissues like the skeletal system, the skin, and joints. These areas often require localized delivery because systemic injection results in the virus being “soaked up” by the liver and spleen before it can reach these tissues.

JointsSynovial Cells, ChondrocytesAAV5, AAV2, AAV8Rheumatoid ArthritisAAV5 is favored clinically because it is less neutralized by antibodies in human synovial fluid than AAV2.

SkinKeratinocytes, FibroblastsAAV8, AAV2 (modified)Wound healing / ScarringAAV8 is highly efficient for fibroblasts; AAV2 often requires “peptide-display” modifications to enter tough skin cells.

1. Targeting Joints and Cartilage

For arthritis or joint injury, researchers usually perform Intra-articular (IA) injections.

  • The Barrier: The joint space contains synovial fluid which often carries high levels of AAV-neutralizing antibodies.
  • The Choice: AAV5 and AAV8 are generally superior for deep penetration into the cartilage (chondrocytes), while AAV2 is excellent for the surface-level synovial lining.

2. Targeting Bone and Bone Marrow

Targeting the “skeleton” usually falls into two categories:

  • Fracture Repair: Local injection of AAV8 or AAV-DJ into a break can stimulate bone growth.
  • Stem Cell Therapy: AAV2 and AAV6 are the preferred vectors for transducing Bone Marrow Mesenchymal Stem Cells (BMSCs) in vitro before they are re-implanted into a patient.

3. Targeting the Skin (Cutaneous)

The skin is difficult to transduce because the outer layer (stratum corneum) is a literal wall.

  • Delivery: Most skin therapies use intradermal injection or topical application with chemical enhancers.
  • Serotypes: AAV8 is often cited as the most efficient for skin fibroblasts. For the outer layer (keratinocytes), researchers use a “shuffled” capsid or AAV2 with specific peptide insertions that help it “stick” to skin proteins like integrins.

4. Vascular Endothelium (Blood Vessels)

Because the endothelium is the first thing a virus touches during an IV injection, you might think it would be easy to target. However, the virus flows by so fast that it rarely has time to bind.

  • Engineered Solution: Scientists have created “venous-targeting” AAVs by adding small peptides to the capsid that act like “hooks” to grab onto the vessel wall.
